A 20 year-old, gravida 1 para 0 woman who underwent an orthotopic 1iver transplantation in 1994 delivered a male infant with a birth weight of 1532 gm after 31 weeks of gestation, via a caesarian section at Aegean University Medical School Hospital on October 11, 1995. The mother had experienced two hepatic coma, three eosephagus varices bleeding episodes secondary to liver failure since 1991. Liver failure was demonstrated to be secondary to auto immune cirrhosis in 1992. She was put on a regimen composed of azathioprine + cyclosporine A + prednisolone to prevent rejection and discharged on postoperative 32nd day. The conception is the second earliest to date, 5.5 months after transplantation. She experienced both mild increases in the levels of liver function tests {ALT 45-150 IU/L, total bilirubin 3-4 mg/dl), and also moderate preeclampsia during pregnancy. The only fetal complication due to maternal immunosuppressive treatment was prematurity and he was discharged at 21 days of age on full oral feeds and is in good health with appropriate growth and motor—mental development as of January 1996. The mother has been experiencing moderate cholestasis since second trimester without rejection.
